 Now you can print out our information form and

bring it with youto your appointment.

Please note that new King County Health Dept. regulation requires all pets be up to date with Rabies shots. According to the new rule we can not groom your dog without it. We don't need to see the pet vet records, but you will have to provide us with an expiration date.

 If you are a new client you can write that information on our client general information sheet below. Existing clients will be asked at their scheduled appointment to provide this information upon their next visit.
